Hi Jennie,

I recommend Para-Plus Translations (http://para-plus.com/). I used this
agency several times to translate documents for me both in private practice
(informed consent forms, health information) and also when I consulted on a
research project that involved participants that spoke various different
languages, they were able to translate survey instruments. They will
prepare documents with formats, fonts, logos as specified. They are very
responsive, professional and located in the Cherry Hill area.
