Sometimes the simple solution is to open the generated PDF in Acrobat, and "save as". This resolves the icon issue and other non-Acrobat generated glitches. As an aside, there few things more annoying than a password-protected PDF. Unless it's a state secret, why bother? One of my clients (a gov't agency) invariably provides me with source documents that come with a password code. This only leads to a hunt and chase to get the unlocked version and ends up costing me time and them money. Although I guess this cumbersome process does keep government workers busy. Sort of like when you see five city workers standing around a pothole, and only one digging.
