All rules apply to all messages. Therefore duplicate messages are created.

In Outlook, and other email programs as well, it is often very
important to have the filters in the correct order.  Most email
programs go through the list sequentially, so if the first rule
filters a message, a later rule can't.  It's sometimes difficult to
determine the right order to get them all sorted correctly.  You'll
have to look at the ones that are being missed and see if they have
anything in a prior rule that made them go to the wrong place.
