For the last nine months, in print, at the Residency Leadership Summit and at
other venues, I have indicated that there will be a time when we need your
voices to be heard. Now is the time.
Last week, the ACGME Board of Directors formally changed the Common Program
Requirements to allow specialty review committees the flexibility to specify
what they need for dedicated faculty teaching time or to stay with .1 FTE/core
faculty of dedicated time. This type of action from the ACGME is unprecedented
and is a direct result of the appeal that we sent on behalf of the specialty
last October.
But the work is not complete. Moving quickly, and with guidance from ACGME
leadership, the Family Medicine Review Committee has proposed going back to the
rules family medicine had pre-2019 with an average of .6 FTE/core faculty
inclusive of precepting and time spent in supervision of hospital care and
deliveries. Here is the specific proposed language:
* II.B.4.d) Core physician faculty members in programs with an approved
complement of 13 or more residents should devote at least 60 percent time (at
least 24 hours per week, or 1200 hours per year) to the program, exclusive of
patient care without residents.
* II.B.4.e) Core physician faculty members in programs with an approved
complement of 12 or fewer residents should devote at least 40 percent time (at
least 16 hours per week or 800 hours per year) to the program, exclusive of
patient care without residents.
* II.B.4.f) Core physician faculty members should devote the majority of
this professional effort to teaching, administration, scholarly activity, and
supervising resident patient care within the program.
Over the last three years, much has been written about faculty time dedicated
to education. What the ACGME needs now is your opinion — agree or disagree — on
this specific policy. Do you prefer this new proposed policy, or do you want to
maintain the current ACGME guidelines of .1 FTE/core faculty member? Other
aspects of the new standards are not up for discussion; they will be effective
July 1, 2023.

possible. From my point of view, all of us need to weigh in — as program
directors and coordinators, as faculty and residents and as organizations. I
also believe that this is likely to be the last chance we will have in many
years to adjust the time our faculty needs to do the work the major revision of
residency standards requires.
