Interesting. The theory about the boiler failing 'due to the sudden 
ebullition of steam when the firebox was flooded' was extensively 
investigated in the 19th century by the Manchester Steam Users 
Association, and they decided that there wasn't enough over pressure to 
cause a failure (by practical experiment!) See Hewison's  Locomotive 
Boiler Explosions.
Certainly the fusing of lead plugs hasn't stopped accidents on big 
engines, and the reports on wartime USA engines suggest some bad design of 
the plugs.

But of course, those were the days where Stroudley, speaking about a 
boiler explosion, said ' I think that if it had not collapsed, the plate 
was thick enough to stand a pressure of 200lbs to the square inch'
