Dear All,
First, I would advise all to go through "IAP Guidebook on Immunization" by Tanu 
Singhal, YK Amdekar & RK Agarwal (2009) which has all the recommendations of 
IAP's Committee on Immunization, published by Jaypee brothers, New Delhi.
The salient features are:
1. Second opportunity for Measles vaccination is definitely required in India.
2. We are giving measles after completing nine months of age. Serological 
conversion is about 80% but vaccine efficacy comes to only 60-80%.
3. Immunogenicity is better if measles is given at 12-15 months of age. But in 
India we see significant number of cases below one year of age.  
4. Second opportunity of measles vaccine is required at two years of age.
5. Government of India is considering introducing MR (Measles- Rubella) vaccine 
in public health schedule. MR will be around 70% economic with added advantage 
of rubella in it. It will give second opportunity for measles.
6. GOI is considering to give this MR vaccine in SIA mode like polio but every 
three years.
7. ICMR has submitted its scientific report to GOI.
8. The targets are: Reduce measles mortality by 2/3 (by 66%) by the end of year 
2010 ( as compared to year 2000). Achieve 90% coverage in 80% of the districts 
by 2009-2010 all over country.
9. Some state governments are giving MMR but the coverage is low.
10. Presently MMR is available in private market and the coverage is very very 
low. 
If the coverage of rubella in community is less than 80%, there will be shift 
to right of the rubella disease and cases of CRS are likely to increase.
11. IAP has already recommended for MMR to be included in national schedule.

Dear friends
MMR is given regularly by the Bombay municipal corporation as a part of the  
regular immunisation schedule since the last 2 yrs and is given free of cost to 
all beneficiaries attending the municipal dispensaries and health posts.

Dear Friends
It is a shame that the Govt Immunization schedule does not have MMR included in 
it. During the elections in this country no leader addresses the health aspects 
as voters do not want / vote on those grounds.
But, not withstanding data or lack of it, as pointed out by one friend, Cong 
Rubella Syn is seen with enough freqency (say as low as one case/ pediatriican/ 
year) to warrant routine MMR vaccination, much before HIB as is being touted in 
some circles.
