I thought that the 3GS came with a ear bud set with a mic on the cord.  My
nokia phone has this on the ear buds and it works great for answering and
ending calls as well as talking handsfree.  

Maybe it is under the accessories page that apple has under
apple.com/accessibility.  

I've not checked out this sight yet but maybe it might have suggestions for
accessibility devices that will make your IPhone experience even better.

I don't want to put in those earbuds for every call or walk around 
with the earbuds on all the time in case somebody calls me.
So if I can't hear voiceover say "answer" and "end call"
I  just double tap until it answers or hangs up. Usually, though not 
always, it will eventually hang up 
anyway. A couple of times I haven't managed to answer till the third or 
fourth ring but I haven't actually missed a call this way yet. What we 
probably need is an independent way to turn up voiceover without having to 
get it to talk to turn it up. I wonder: if you had a real headset with the 
microphone built on instead of just the earbuds, is there an auto-answer 
setting? Or could you get a headset with an answer button on the headset? 
These might be options if you didn't mind wearing a headset all the time, 
though I think I'd rather not do this.
