Yes the playlist has to be created before you move anything to it.
all the current play list will be listed in the submenu.
Also, by "higlight", do you mean interacting with the outline and
going to the song. Then don't stop interacting or hit enter or
anything. Just do ctl-option-shift-m.
You you should be interaccing with the list. It is also best to have
the mouse following the VO cursor.
Finally, I don't think the menu comes up at all if you are playing
the selection.
It does come up when playing for me.
