Hi

So far that I know, epub is no issue with the ibooks application.
You can simply read pdf files with preview on your iphone / ipad.
Preview supports also .docx and .rtf files.
If you want that the app saves the location in the file where you stopped 
reading, fileapp pro should be something for you.
You can easily transfer books from your computer via ftp through your iphone / 
ipad into the fileapp pro application.
Fileapp pro supports .docx and .rtf and maybe, but correct me if I'm wrong, 
pdf's. Don't know if in the last format your location where left is saved. More 
file extensions are supported by fileapp pro such as power point, excel and 
such.

there are voice over issues with the kindle application what I've red on other 
lists. Kobo and stanza are other opportunities
 to read digital books. The B&M Ereader seems to be accessible with voice over, 
but never tested in to death.
there are procedures to convert scanned books to epub. The steps how to do this 
are mentioned some time ago on this and other lists.
