How much RAM does it have 2Gb or more
The recent updates recommend a minimium of 4Gb

Are you using only one voice and is it Alex ?
Open and then close voiceover utility. This may trigger a refresh.
Close down your mac and fully detach the power supply, that is, take the
power cord out of the wall for 30 seconds.
Are you running a lot of Apps at thre same time, close them and see if
there is an improvement.
