No, it doesn't happen when using any particular app. It happens any time the 
phone is unlocked and voiceover is active. If I triple press the home button to 
turn voiceover off, the speaker goes out in a few seconds. The same thing 
happens when I lock the screen. However, if I do a three finger double tap to 
turn off speech, the speaker still stays on. If I leave the screen unlocked and 
just sitting there doing absolutely nothing, the speaker will not turn off.

Just gave it a try, couldn't tell that the speaker was hot. Will try again in a 
quieter environment when I get a chance though.

I seem to get about the same battery performance as my non-voiceover using 
friends and family.

Is there a specific app you're using that you notice it on?

I'm wondering if you have some sort of raise to speak option enabled, so that 
when you put the phone to your ear the accelerometer and proximety sensor wake 
the phone up. Sort of like the light in the fridge that comes on every time the 
door opens.

BTW, my understanding is the list is for all things Voiceover.
