Okay, managed to get it sorted out.  Had my sighted wife help me.  I
apparently have two mentions of my iPhone in the devices list and I needed
to go to the second one where the four boxes would appear.

When you open Itunes, go to the sources list, and select your iphone as the
source.  Then tab twice and the code should then be accepted.
