I don't know why, but we're almost never told anything about what Apple does/fixes with VoiceOver. In fact, we almost never hear anything about VoiceOver at all until a major version shows up on a new OS. VoiceOver certainly changes with OS updates, but the people in Apple accessibility either won't, or are told not to, keep anyone in the loop about what happens with VoiceOver. When a new OS patch comes out, it is entirely on you to push every button, dig through every menu, re-try every app and see if you can discover a change. I think that's wrong. I don't know why VO changes can't be listed along with an OS release. For example, every last freakin' developer release of 10.6.4 talked about VO changes, particularly as affects the iLife applications. However, when the release of 10.6.4 comes out, the only mention of it is, wait for it, support for a few more Braille displays. That's it? Why generate all of that hype along the way, and not talk about it at the end? Who knows. Probably a lot of hard work went in to updating VO to fix some tough problems. Too bad that most of it will go unnoticed, because no one knows where to look. People won't even know that a previously inaccessible app is working well now until someone decides to beat their head against a brick wall a few months from now, and is pleasantly surprised to find that Apple fixed that months ago.
