you don't have to use a headset, just lay the phone flat and the speaker will activate and then you can access the controls.

No: they expect you to use the iPhone with it's headset That's all. Because then you have access to the phone's full functionality, e.g. adding an appointment to the calendar, then coming back to the phone app etc.
