That's actually an OS X issue, what happens when you don't have a mouse hooked up is that the bluetooth setup assistant launches first thing, assuming you want to connect a bluetooth mouse. Unfortunately, at that point the rest of the system isn't running so Voiceover won't come up. You can fix this, open system preferences/Bluetooth and hit the advanced button. Uncheck the checkbox labeled "start bluetooth setup wizard when no input device is present" and the press ok and close system preferences. That will prevent it from happening again. It should be noted that the bluetooth setup wizard is accessible in all other cases, just not when it fires up that early in the boot process.

I only want to say that, at least on the newest version of this model, VoiceOver will not work unless you connect a mouse to one of the USB ports. At least that's what happened to me. It worked just fine during the setup assistant, but once the system booted completely I had to plug in the mouse for it to work.
