In all of the British fiction I've read over many, many years, the quotes in
dialogue come before the period. The opposite seems true in US fiction. The
rules for ? are self-evident.

I don't know about ! though. I doubt that the British write:

"Help"! he said. 

But I have no examples in front of me.

I've been away and unable to begin to correct the various grammatical blunders
and solecisms in this thread until now.
