Medicare does pay for out of state treatment as long as in US.  Re out of
the country, I think they pay in Ontario, Canada and Mexico.  I don't know
about Europe, Asia, etc.

effect national (federal?) as some of ours are, but I assume only some
categories of people can be treated in your top ones.
