Mike,

It looks like you forgot to attach the tables. I'll add them to the 
liblouis distribution. Most of the problems with the Nemeth table are 
probably in the table itself. I'll have a look. the root problem is due 
to the fact that in MathML the index of the root is the second item, not 
the first. This will indeed take some work on the code, probably in 
liblouisxml.
