It causes a space to be placed before the text in the em tag. Without it there are a lot of words run together in Bookshare files. That is messy, because it makes liblouis think that there is an ialicized part of a word. I don't know if it is necessary in nimas.sem . nemeth.sem uses the third column a lot. You can see how it is used by looking at transcribe_math in transcribe_math.c .
