Keith, to illustrate what Doug mentions below, this what it should look like. (Telyt-R # 2494419, even earlier than yours) http://gallery.leica-users.org/v/DMS/New-Old-Pictures/_MG_6138-Edit.jpg.html Top left (ringed in red) is the 3rd Cam (which isn't really a cam at all) this is installed parallel to and inside of the sloping cam at the left, the raised lug at the right hand end of this black painted, stepped piece of metal should protrude above the surface of the actual cam (unpainted) by about 0.5mm at the end of the cam, this lug should be parallel to the cam and not bent out of shape. The other, identical, cam at the right is the second of the "2 Cams" found in all Leica-R 2 and 3 Cam mounts. The bottom left is the aperture control lug. If you set your lens to f22 and turn the aperture ring clockwise the "2 Cams" and the extra "3rd Cam" move clockwise in unison. The aperture lug moves proportionally as the aperture opens - in fact it hardly seems to move at all between f22 and f11.
