creating a blank form template with word 2010 and jaws

hi.
using jaws 11.
the latest build.
what is the keyboard short to create a template.
like a blank form template.
any one got any ideas.
or how to use this.
did find a web page with a list of keyboard short cuts.
looked through the sample templates.
but cannot find any where.
if i press alt f, then control tab to new.
then i tab to the list of templates.
hit f twice, till i hear forms.
which is not selected, then press the space bar to highlight.
a list comes up.
but not the blank template for the form.
then you get the create button.
and also keyboard short cuts to add controls to a form,
edit boxes, or text boxes, and check boxes.
please help me out.
