To change how JAWS  reads through a document: go to "change accessibility 
options" under the "document" menu heading. There, you will find the radio 
buttons that read entire document or just the visible page.

  As far as I know it's not possible  with jaws but acrobat should have a
  read aloud option. It  should be somewhere in the tools menu. Then if
  you have sappi installed sappi then read the whole document at once but
  the problem is: it will read all punctuation as well.
