Forms in PDFs that are not made accessible by the author can be pretty tough to fix by the Jaws user (unless they have additional software and sighted assistance). If the author used a tool called CommonLook for PDF, they could make the PDF accessible rather quickly. But, if you need a fix for now, you can try the following. Use changing Jaws options for finding the right text for each edit box. This will basically allow Jaws to look to the left, right, up, and down from the edit box to find the associated text. Note, you will not know for sure that the given text is truly associated with an edit box without sighted assistance. That said, change the Jaws Verbosity to try and get Jaws to automatically figure out the label. While in the PDF, hit INSERT + V. Then arrow down to "Form Fields Identify Prompt Using", and change from "Label" to "Label and Titles if Different". If this does not work, try some of the other options such as "Label and Alt if Different". You could also try the Read Out Loud tool, and adjust it to read forms (see previous email where I discuss changing Read Out Loud Preferences). Sometimes it help, but sometimes does not.
