Cy, IF you landed on "Manage Applications Settings" from the JAWS dialog (Insert+J, then O, then M) then it's already stuck, as long as you pressed the "OK" button after checking the Outlook 2007 item. If you found "Manage Applications" somewhere else, then I'm clueless. When looking at an email in Outlook 2007 (W7, 64 bit) and you exit the email focus seems to want to go to the first of the email list.
