Eileen, There are four buttons that specifically say: Password/access code, return code and two continue buttons. The entire screen said: Seattle City TOP frame Images/SClogo TOP frame end sidebar frame sidebar frame end survey frame Seattle City Light and Right Management would like to welcome you to the 2007 Employee Training Assessment Survey. The questions presented in this survey will help City Light assess your comprehensive training and development needs. We would like to build on the previous employee survey where you identified training as a top priority for the Utility. Employees at all levels of City Light are encouraged to provide their feedback on the survey. Participation across the Utility is vital in developing an optimal training program. The survey consists of 18 questions designed to draw on your training experiences and preferences. It should take no more than 15-20 minutes to complete. Please plan on reserving enough time to take the survey in one session. In the event you are unable to finish the survey in one sitting, you will be provided with a randomly generated return code that you can use to come back and finish the assessment. The survey will be accessible online for a three-week period from November 3 to November 30. All responses must be submitted on or before November 30. This questionnaire is completely anonymous. Right Management will be collecting and compiling all the data and no one from Seattle City Light will see any individual responses. The questions are easy to answer. Some will ask you to rate how much you agree with a particular statement, others will ask you to ?rank? or ?check all that apply? from a list, and others will ask for your comments. You also will be asked to provide some voluntary background information, which will be used to better understand how major groups respond to the survey. Again, we are excited about the opportunity to bring this survey to our employees. We look forward to hearing your feedback! Thank you for your participation!
