I too have experienced this problem many, many times with all of the = modifier keys at one time or another. Along with alternately tapping each of the left and right ALT or SHIFT = or CTRL keys, I have also found the unloading and reloading JAWS = unfailingly cures the problem. This leads me to conclude that it is a = JFW problem. I usually just restart (if possible) and that corrects the problem. I assume its happening because I've hit some wierd key combination by mistake, but I'm not exactly certain.
