it looks like The single caps voice is what you are looking for and you can 
find it in speech and sounds manager under misc.  I haven't been able to get 
single caps spoken since the advent of 5.0 when they went to the voice 
aliases and speech and sounds managers.

I found the control on *when* to indicate capitalization, but not *how* to
indicate capitalization. And where did the spell decrement option go?
