Dave, it is more complex than that.
It also depends on exactly which version of any given release as to whether
or not it might work.
On one updage it might be fixed only to be broken on the next.

Absolutely not correct. Please check your statements. Ctrl+R is *not* the 
same as Insert+Shift+R. As you can see, my message was sent direct to you 
(using CapsLock+Shift+R from my laptop layout) and also to the list.

again I point out that due to variants in the JKM file for laptop vs. 
desktop layouts, and the variant usage of JAWS and Insert  as key modifiers 
in the same file, there is a lot of room for JAWS to act completely 
unpredictable from one person's system to the next. Much experimentation 
using the Insert+1 training mode is suggested.
