 Dear Coordinators,

 Please complete the LOG approval form attached to this email by 26th October 
2016 כ"ד תשרי תשע"ז 


 Complete it only for the students and classes who will be completing the LOG 
at the end of this year.


Send the completed form to this email​:​ pattalshir@xxxxxxxxx , which is 
specifically for the​se forms.

 After approving the program the approval form will be ​signed and then kept in 
our records in the office. The forms will not be returned to 
 you.
 ​Please check that the texts on the forms are from the list of approved texts, 
which can be found on the TLC site.​
