If using XP
Hit windows Key d or windows key m for desktop
Press F5 so no items are selected on desktop
Press Context menu key (left of control on the right-hand side) or shift F10
and press w for new and f for folder
Type the folder name followed by enter

create a new folder.
Usually hit one, but, bouncing arround with jaws at moment.
