Web Server publishing rule ignored in favour of default rule

I'm having real trouble trying to publish a web server through ISA 2k4.

Despite following the wizard, Tom Schinder's book and trying various fixes
offered on this list and elsewhere on the web, I've got nowhere - which
generally means it'll be something simple!

I have a webserver on the same subnet as the internal interface of the ISA
Server and while I can access the default home page locally and from the
ISA Server, external access is denied.

On inspection of the ISA logs, the Firewall is simply denying it with the
default rule.  I've tried reconfiguring the web listener to listen on
other ports, checked the routing tables and verified that nothing (as far
as I can tell) else is using port 80, but everything looks fine.

Looking on the Technet Groups, there are a few other people with the same
issue and no-one seems to have found a solution yet.

Any help would be much appreciated.
