Did you ever find an answer to this?
I have a similar problem whereby a web site will work for about a week and
then mysteriously stop working and the ISA Server Web logs shows 12202 error
messages.
Restarting the WebProxy will fix, but I am trying to solve the problem.

From time to time, one of my 4 sites stop working, showing 12202 error
message in browser (403). If I do nothing, in few hours site start
running.Maybe someone can help.
