I don't really know why any of you are surprised.  I mean really.  You take
any technology, dumb it down to the lowest common denoninator and then
shrink-wrap it and stick it on a shelf for $39.99.  Sell a billion of them
and then apologize for it later.  The Wi-Fi industry did it.  That's why
there are 6+ open AP's within range of my house.  'Cause the ^%#%^ don't
work!  It is NOT interoperable.  And it is too freakin hard to configure for
the average schmuck.  So now the VOIP guys all want their turn.  
 
The FW folks are still playing in their own sandbox and not presenting
viable VOIP FW transmission options  If I were a VOIP guy trying to sell the
hot-thing-de-jour, I would be trying to figure out a way around the FW too.
That document is 3 years old and FW like ISA still don't support SIP, etc.
Sad... just sad.
