If this is an anonymous rule (all users), it would also need to precede any 
authenticated rule; allow or deny.

It would need to be before any deny rule that would prevent it from
being allowed.  You're telnetting to 25 from an internal box and not the
ISA box itself, right?  And the internal box either has the firewall
client installed or has the ISA box's internal IP set as it's default
gateway?
