Absolutely, SUS is the way to go. They have even become very good at
making new updates available to it as fast as normal Win Update.

Just for the sake of credibility on that opinion, absolutely no network
design interpretation was required and is therefore likely to be
accurate.

I don't even try.
I use a SUS server internally and that way, each patch gets downloaded
only once and is forever available to all the inside folks,
regardless of the Internet state.

Turns out the catalogue is now downloaded by HTTPS, and so you need to
have
not only a HTTP but also a HTTPS allow out packet filter for the default
external interface of the ISA box, then it works fine.

I have read around a bit about auto updating servers and there seems to
very
polarised views, does anyone else update their ISA box (or servers) this
way, keeping in mind that ISA specific updates will have to be
downloaded
separately anyway, is it worth it ? i.e. you are not going to get
MS04-001
from Windows Update and I haven't seen it on SUS ? But I suppose it's
better
than nothing to at least have the underlying O/S up to date.
