The latest ISA advanced feature pack has not been installed on this Server - 
any thoughts about applying the latest SP?

That fix addressed a very specific set of issues, rather than being an all 
purpose fix. I believe one of the big issues had to do with not fully 
qualifying the requests had some interesting effects or vice versa :-)

Hey simon,
 
This could be happening due to the lack of a ISA Server 2000 patch that 
corrects the Rules Engine. I used to have this kind of issues too. After it was 
applied, no more ;)
