"How long can an Internet-facing Exchange server survive without some form of 
protection?"

Is it necessary to have to ISA server for both end to have RPC over http tunnel 
in order for SecureNAT client to use "RPC over HTTP" 
function in outlook?

What if,

SecureNAT client --> ISA server --> DSL --> Internet --> Exchange

Is it possible?
