Ports Forward to an IP address

Dear all, pls help me to configure ISA to map the following ports to the
IP address of the PC that is using BuddyTalk:
UDP Port 8481
UDP Port 8995
I have created "allow" IP packet filter for UDP port 8481/8995 to forward
port traffic to an internal IP address, but it does not seem to be working
with my NetPCPhone, please shed me some lights how to configure the ISA
server to forward the ports.
