OWA fails from outside my network

I have set up OWA and it works if you connect from inside my network
using internal IP of Exchange 2003 machine.
I have the web publishing rules pointing the 3 Exchange 2003 directories
to the External IP of my ISA box.
I enter http://[public ip of isa box]/exchange and it fails.
Can I use IP instead of a registered domain name?
Could I set up a sub domain like owa.domain.com and use that? If so, I
am not clear on how to set up A or CNAME records in DNS.
All the documents on isaserver.org assume this knowledge and I do not
have it.
Any help will be greatly appreciated.
