I found my problem....
I had "Send the original host header to the publishing server instead of
the actual one (specifiy above)" checked. I removed the checkmark and it
appears to be working now.

I have a question on getting my OWA working correctly. I can access the
OWA interface from within my private network but get this error from
anywhere else.
"403 Forbidden - The server denies the specified Uniform Resource
Locator (URL). Contact the server administrator. (12202)
Internet Security and Acceleration Server"
Any idea what I am missing on this one ?
Information on Setup:
- E2K on Internal Network
- Published the Web Server
