Clearly, their app is not proxy-aware.
This looks like a job for "directaccessman"!

OK, I know this is WAY OT. But I have further info. I still cannot run this
software, unless I do the workaround listed below. Apparently it uses port 80,
and runs an update.exe file everytime you connect. I see nothing in ISA logs
that show it being blocked (I think). Is this even an ISA issue, or maybe just
a Proxy thing or ISA blocking the exe file from running? Here is the latest
from the "tech support", that does work by bypassing the running of the
update.exe file... this is not important, but bugs me that I cannot get it to
work...Do I need to define a custom protocol for port 80 for this particular
website?

"The update software is run automatically each time you open the client.
The update works as a normal HTTP port 80 web service.

You can bypass this update, which may work for you.

From the Start menu, click Run and type

   C:\Program Files\PokerStars\PokerStars.EXE.

Alternatively you can go My Computer -> C: -> Program Files -> PokerStars and
double-click PokerStars.exe, or alter your desktop shortcut to run this program
instead of the PokerStarsUpdate.EXE that it now runs.

Note that this does bypass the automated updates. Some updates
are mandatory. This should only be used as a temporary workaround."
