[isalist] No Rule?

I know this is something that should be simple, but I cannot recall what it is
that causes it (I haven't had to work on ISA/TMG for a looooong time).
I have traffic that is not hitting any rules. It is generating a 0x80074e20
FWX_E_GRACEFUL_SHUTDOWN error in the logs, but not much other details. Traffic
is from the localhost to another server, the protocol is properly identified, a
rule that should apply is defined, but it never hits the rule.
Anyone have any quick ideas on the cause of this?
