You may have to select "Ask unauthenticated users for identification" in the 
Outgoing Web Requests
listener.
You'll find that setting under <Array/ServerName>, Properties.

Hello Everyone

We use ISA server 2000 with sp1, when we configure a site and content rule
and select applies to users and groups specified below, when we choose a
user or domain group, access is denied to anyone trying to access the
internet via the ISA server. It works with client address sets and any
requests.
