ISA 2004 SMTP Config Question

Looking through Tom's notes about how to setup Exchange SMTP on ISA 2004
he gets to a point were he uses a "Perimeter Relay" which he gives the
IP of the SMTP Relay machine which is in his diagrams.
I guess the million dollar question is what if someone doesn't have a
SMTP Relay machine?
