I just checked with some other users here (I don't have a Hotmail
account), and Hotmail works just spiffy.  I have no Direct Access rules
at all, all clients are using FWC, and authentication IS required before
getting through.

on SecureNAT configuration.

I just checked my rules, I have nothing special set up for hotmail etc,
just a general allow http / https from internal to external. All my
other rules are setup for various servers and users eg ftp.

Only direct access sites I have configured are the windows update sites.
