My situation matches exactly this scenario.
I had disabled websense and still seen the error on ISA, so assumed it was not
websense related.
(and their website revealed no answers)
Has anyone else seen this?

Are you behind websense?
If so I and several others never got it to work correctly. Neither Microsoft
nor websense were ever able to come up with a solution (or even really
acknowledge that there was a problem), but it happened to 100% of the schools
in our region that were behind websense. We finally fixed the problem by moving
to 8e6 for filtering. Now ISA 2004 works like a charm.
