Destination Sets for Server Publishing

Can someone please explain destination sets for server publishing.  I want
to test ISA Server without using FQDN.  In Tom's article on web publishing
he states that you should have destination sets and imputs the following:

www.domain.com
ftp.domain.com
mail.domain.com

We don't have External DNS configured yet, once configured will it even
know what domain the request is coming from?

We have to look at destination sets coming from the external users
correct.  I can undestand imputing IP address' to limit user access to a
web site via IP, but the wev sute we have, we want open to everyone.

I'm just really looking for a complete picture and understanding.  Thanks
