I have mentioned the real message that is Filling my Application Log 
continuosulsy.

Packet filter address is changing on each time.

"The ISA Server services cannot create a paket filter 216.127.42.66.
This event occures when there is a conflict between the local address
table(LAT) configuration and the windows 2000 routing table. Check the
routing table and LAT to fine the source of the conflict."

Please help in to sove that one.

Remember 

Every Thing on Client End is running smoothly. According to me! I have 
construct the Table

on the basis of Windows Routing Table.
