there are other services to consider as well, Like DNS, do you know the
structure of your provider's DNS, are you primary for your name space or
does your provider manage that piece for you, have you done simple
NSLookup's from your ISA server to places people visit on the Internet, how
long does it take to resolve? Just a suggestion. you might perform a simple
Trace Route as well to see the response times and hops to your final
destination, is your provider a tier One or Two or Three .. Etc ... things
like that need to be understood as well, does your Internet service provider
afford you a preferred route or do they handle routes off your network
through load balancing?
it all factors into performance. Generally speaking, I have setup ISA
servers and found that out of the box with little or no tweaking it cooks
along very well and the response times from Cache are very acceptable. I
would look deeper into your network and beyond to your provider. Remember
down stream is as equally important and often over looked.

    If you think it might me a slow connection you could use mrtg to monitor
your bandwidth utilization.  It is open source and a very good piece of
software.  Plus runs on anything.

Fine, but what about your pipe to the Internet, maybe you should request a
traffic study from your provider to see the utilization on your bandwidth.

Do you know what kind of connection you are using?

I have looked at Performance Monitor and everything seems to be running 
within "norms" which is why I am a little baffled at the perceived 
performance. 

The page file is 2GB on the system drive. 

It is Windows 2000 Standard.

While increasing the CACHE file size will help, have you looked at 
Performance Monitor to see if there is a bottle neck somewhere on the 
server? 

How is the page file set? 

Is this Advanced Server, and if so, have you set the large memory switch 
in 
the boot.ini?
