All SUS traffic is either HTTP or HTTPS.
Since you're not authenticating outbound requests, you shouldn't be seeing any 
issues conencting to your SUS server through ISA.
Q1 - How have you configured the SUS clients; GPO or registry?
Q2 - What are the settings you've applied?
Q3 - Do you find any SUS requests in teh ISA web proxy log?

Q1 - Our ISA server is not internet facing.
Q2 - We support Windows 2000 and 2003.
Q3 - Currently we are using integrated for incoming traffic. We have
exchange servers plus domain controllers etc.. behind the isa server to
provide OWA. SO there is no need for any traffic to go out through isa
except dns, smtp and sus update and no authentication for these.

I wonder if anybody as manage to allow SUS update through the ISA
server.
I have setup protocol rule to allow port 80 but it is not getting
through.
